Monthly installments are for Family Passes only. No other memberships are
eligible for this plan. 2012 Family Pass cost is $612.37 including sales tax.
There is a $10 one-time fee for using the ACH Service, so the total cost is
$622.37 plus any additional special items you add like the babysitter costs.
if you add the babysitter to the family pass, cost is $665.63 plus $10 ACH fee,
making the costs $675.63.
Divide those costs by 4 and that will be your monthly deduction.
This form must be filled and returned to the address listed on the form. All
forms must be turned into the pool via mail no later than Monday, February 20,
2012 in order to participate in the monthly withdrawals. Payments will be
deducted from your checking account on the 28th day of each month for four
months beginning February 28th, 2012.

Please allow 7 business days to cancel or remove yourself from the plan. There
is a $25 processing fee to remove yourself from the plan once the deductions
have begun.
